So here's the question: any thoughts on a magazine extender? The pinky finger just barely fits, and the palm is half hanging loose on the other side. Having something that would drop down a bit would make it easier to grep.

Yes, purchase the 12 round magazine. The one pictured is with the 10 round. That will give her the grip she needs.
Purchase the 365XL grip. Yes, you'll need the longer magazines to go with it, but I've got pretty good sized dick-beaters on me and the XL frame works perfectly.
JDP, it looks like SIG makes a 15 round one, too.

Buddy of mine has one. It’s the NRA version. Came with a couple mag extensions and appropriate baseplates. JPD has it right. Not sure you have to buy entirely new mags, maybe just a 12rd conversion kit. I’d buy the spares anyway.

It's super easy to remove your current 365 grip module and replace it with the XL grip module. You'll need to get the bigger magazine to work in the new XL grip module-

The new magazines usually come with an installed baseplate, plus an extra one to fit on the XL grip module.
