Switch probably has spanning-tree enabled for the port, with portfast disabled and the port is not set as an edge port. Either set the port as an edge port, or enable portfast. I don't recommend disabling spanning-tree. I recommend enabling portfast.
Also, 1.3.65 Intel PXE boot agent for gigabit ethernet NICs is a little old. 1.4.04 is the currently released version If this is a LOM then get an updated BIOS from your motherboard vendor, or if it's a plug in NIC, download driver release 17.4 from Intel and update the boot agent.
On the gripping hand, that MAC address has either been spoofed, or someone has somehow put the Intel boot agent on a Pegatron NIC, so that may also be the reason it's not working...

And Broken Andy, if you want to PXE boot using IPv6, then you want to UEFI boot, not legacy boot.
