Gun safety as we know it today really didn't get invented until the 1980s or so. It's a fairly recent concept.
Watch film footage of currently deployed US soldiers in the sandbox versus Vietnam-era combat footage. The muzzle and trigger discipline of even US Army infantry units was appalling as recently as twenty-some-odd years ago.
